Job Available:

Direct Support Professional

The direct support profession is responsible for meeting the needs of the consumers by providing direct care and supervision, positive behavioral support, and instruction consistent with the needs and individual program plan (IPP) of the individual. The DSP is responsible for the safety of the consumers and maintaining a living environment that is clean and homelike.

Dress Code: All staff are required to ware medical scrub attire at all times. Uniform must be clean and iron whenever on duty.

Qualification:

  • 18 years of age
  • High School diploma
  • First aid and CPR equivalent
  • Criminal fingerprint cleared by FBI/DOJ
  • Current health screening and TB clearance
  • Ability to speak and understand English
  • Good physical health and mentally capable of performing assigned tasks
  • Knowledge of how to recognize signs of illness and the need for medical intervention
  • Knowledge of community resources
  • Employer references and/or letters of recommendation
  • Complete and successfully pass a 35 hr. DSP1 and DSP2 course
  • CPI or PRO ACT certified w/in 30 days

Duty Statement
Hours/Shifts: M-F 6am-9am, 3pm-10pm, 10pm-6am, S-S 6am-2pm, 2pm-10pm, 10pm-6am

  • Assist with personal care and grooming
  • Limited meal planning and preparation
  • Assisting consumers with making appointments
  • Escorting consumers to appointments
  • Household chores including laundry, deep cleaning, lawn upkeep
  • Shadow resident if one to one is required
  • Maintain confidentiality
  • Observe and report changes in consumers on an on-going basis
  • Charts services provided for each resident on daily progress notes
  • Attend Monthly staff meetings 1x per month
  • Follow rules for attendance, reporting, etc. according to the facility guidelines
  • Attend in-service trainings and programs as assigned to keep service hours updated
  • Perform other duties as assigned
  • Record consumer’s medication in long
  • Supervise consumer activities
  • Assist in preparation of daily meals
  • Familiarity of community resources that the consumers may request or need
  • Assist in dispensing consumer’s medication
  • Maintain inventory of supplies

  • Complete 12 hours of continuing education per year to maintain certification

Lead Direct Support Professional

The Staff Leader oversees the day to day operation of facility and care and supervision of all consumers and oversee of all personnel staffing schedules. The Staff Lead is responsible for implementing the operation plan, procedures and maintaining all consumer files, medication administration records, staffing schedules and supplies.

Qualification:

  • 21 years of age or older
  • High school diploma or GED equivalent
  • First aid and CPR certified (with abdominal thrust technique)
  • Criminal fingerprint clearance for CCL
  • Current health screening and TB clearance
  • Licensed to drive in California/Valid Insurance
  • Knowledge of laws and rules pertaining to Adult/Elderly Residential Care
  • Capability to maintain consumer and employee records
  • Possess good character and reputation of personal integrity
  • Successful completion of 35 hour initial Administrator Certificate that is current and in good standing. Administrator must complete at least 40 hours of continuing education during the subsequent 2-year period.
  • The Staff Lead must have a minimum of six months of prior experience providing direct supervision and special services to persons with developmental disability
  • Within 6 months of becoming an administrator, the individual shall receive HIV, AIDS, and tuberculosis training. Thereafter the administrator has to receive updated training every two years
  • CPI or PRO Act certified

Duties and Responsibilities:

  • Responsible for implementation program for all services including program needs, food, transportation, laundry, etc.
  • Responsible for compliance with Title 22 Community Care Licensing regulations and communications with CCL representatives.
  • Responsible for compliance with all city, state, and Federal laws governing abuse reporting, consumer care, and labor laws
  • Responsible for reporting all employee disciplinary recommendations to Administrator
  • Responsible for implementation and documentation of all staff training
  • Responsible for care and supervision of consumers according to the consumer’s needs as documented in the pre-admission assessment
  • Responsible for ordering all supplies and items needed to perform day to day operations
  • Responsible for ensuring that nutritional and health needs of all consumers are being met
  • Possess the physical energy and competence to provide care and supervision
  • Liaison with the social service agencies as needed
  • The Staff Lead is responsible for the overseeing staff scheduling and call off’s
  • Responsible for the timely submission of special incident reports

Salary: $14.00-17.00 per hour

Administrator

The administrator has the ultimate responsibility for the care and supervision of all consumers and supervision of all personnel. The administrator is responsible for developing an operation plan, procedures and budgets, as well as maintaining all financial, consumer, and employee records.

Days and Hours on Duty
The administrator/licensee is on call 24 hours a day/7 days a week.
Mandated 20 hours: MW 8AM-12PM; TR 2PM-6PM; S-S 10AM-12PM

Qualification:

  • 21 years of age or older
  • High school diploma or GED equivalent
  • First aid and CPR certified (with abdominal thrust technique)
  • Criminal fingerprint clearance for CCL
  • Current health screening and TB clearance
  • Licensed to drive in California
  • Successfully completed the regional center orientation
  • Knowledge of laws and rules pertaining to Adult/Elderly Residential Care
  • Capability to maintain consumer, financial, and employee records
  • Possess good character and reputation of personal integrity
  • Successful completion of 35 hour initial Administrator Certificate that is current and in good standing. Administrator must complete at least 40 hours of continuing education during the subsequent 2-year period.

  • Responsible for development, implementation and documentation of all staff training and maintaining current staff appropriate certifications
  • Responsible for care and supervision of consumers according to the consumer’s needs as documented in the pre-admission assessment
  • Responsible for assessment and re-assessment (LIC 608) of all consumers, including interview with consumers and responsible parties
  • Responsible for ensuring that nutritional and health needs of all consumers are being met
  • Ensure that consumers have opportunity to participate in community activities that promote wellbeing and independence
  • Possess the physical energy and competence to provide care and supervision
  • Liaison with the social service agencies as needed
  • Responsible for community outreach to physicians, hospitals, social workers, referral agencies, and local community groups to promote the facility
  • The administrator is responsible for the implementation of the consumer’s goals
  • Schedule, reschedule, cancel and maintain all physician/therapy appointments to maximize the best care practices for the resident
  • Responsible for the timely submission of special incident reports
  • Maintain strict confidentiality of personal health information
  • Maintain a positive and flexible attitude, cooperate effectively and collaboratively to help others

Knowledge/Skills/Abilities:

  • Knowledge of CCL regulations Title 22 and Title 17
  • Ability to implement and define scheduling protocols
  • Strong Customer service skills
  • Knowledge of Microsoft word, excel, outlook calendar software

Licensed Vocational Nurse

Licensed vocational nurses provide care base on care plan from nurse consultant and or physician and IPP. LVN may read vital signs such as pulse, temperature, blood pressure and respiration and glucose testing. They administer injections and enemas, monitor catheters and give massages or alcohol rubs. They may apply dressings, hot water bottles and ice packs when necessitated. Additional job tasks include watching patients closely and report any signs of treatment complications or adverse medication reactions. LVN’s will be contracted on an at-will, as needed basis when warranted by physician’s care plan to necessitate appropriate care beyond that which can be provided by a DSP.

Duty Statement Hours:
MW 5pm-7pm or as needed to provide adequate care necessitated by consumer

Registered Nurse

Licensed RN provides care plan in conjunction with the physicians’ orders and the IPP. The RN consultant will evaluate progress of care needs for the consumer on an ongoing basis and prepare quarterly reports on health conditions.

Duty Statement Hours:
Level 4I a minimum of 10.66 hours per month or 64 hours within a 6th month period

Duties and Responsibilities:

  • Collects data pertinent to the healthcare consumer’s health or the situation.
  • Provide initial assessments of resident upon entry or re-entry into facility.
  • Accompany Administrator to interview potential residents to identify if facility can provide level of care.
  • Analyzes the assessment data to determine the diagnoses or issues.
  • Identifies expected outcomes for a plan individualized to the patients or the situation.
  • Develops a plan that prescribes strategies and alternatives to attain expected outcomes.
  • Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment.
  • Evaluates progress toward attainment of outcomes.
  • Provide in service training to care providers and family as needed
  • Attains knowledge and competency that reflects current nursing practice.
  • Integrates evidence and research findings into practice.
  • Collaborates with healthcare consumer, family, and others in the conduct of nursing practice.
  • Prepares quarterly reports in January, April, July, and October on consumer’s health care condition.

Qualifications: (At Will Employment)

  • 18 years of age
  • Graduate from an accredited School of Nursing required. License to practice as Registered Nurse or temporary Registered Nurse petitioner in the state of California required.
